On Sept. 5, Wongel Estifanos and her family visited the Glenwood Caverns Adventure Park. That evening, the 6-year-old Estifanos died on the park’s Haunted Mine Drop ride. On Thursday, state officials submitted results of an investigation to 9th Judicial District D.A. Jeff Cheney.
